Program Forms and Waivers

The foundation of BLUR's mission is to help young artists develop their talents in a nurturing environment. To achieve this mission, we must create an atmosphere in which safety and mutual respect for one another are ensured.

BLUR's Student Handbook details all of the policies of the program and provides important information for students and parents. Also enclosed in the Student Handbook are three forms that must be printed out, signed and returned to BLUR NO LATER THAN June 1. Students will not be allowed to take up residence in the dorm unless these forms are signed and on file with us.

The three forms are the Student Commitment form, the Parent Commitment form and the Medical History form. If you are taking prescription medications or over-the-counter medications, you must also fill out the Medication Authorization form. (Please click on the bolded words to download the forms.)
